Opioid use and misuse in children, adolescents, and young adults with cancer: a systematic review of the literature
Beauchemin, Melissa ; Dorritie, Richard ; Hershman, Dawn L.
Adolescents and young adults (AYAs) are at increased risk for negative opioid-related outcomes, including misuse and overdose. High-quality cancer care requires adequate pain management and often includes opioids for tumor- and/or treatment-related pain. Little is known about opioid use and misuse in children and AYAs with cancer, and we therefore conducted a systematic review of the literature using PRISMA guidelines to identify all relevant studies that evaluated opioid use and/or misuse [...]

Screening Tools for Identifying Older Adults With Cancer Who May Benefit From a Geriatric Assessment: A Systematic Review
Garcia, Maja V. ; Agar, Meera R. ; Soo, Wee-Kheng ; To, Timothy ; Phillips, Jane L.
Importance : Older adults with cancer are at risk of overtreatment or undertreatment when decision-making is based solely on chronological age. Although a geriatric assessment is recommended to inform care, the time and expertise required limit its feasibility for all patients. Screening tools offer the potential to identify those who will benefit most from a geriatric assessment. Consensus about the optimal tool to use is lacking. Objective : To appraise the evidence on screening tools used [...]

Sex bias of females in survival from cancer and infections. Is X the answer?
Emran, Abdullah Al ; Gallagher, Stuart J. ; Tiffen, Jessamy C. ; Hersey, Peter
Major differences in survival of men and women from infectious diseases and cancers have been highlighted by death rates from COVID-19 infections. In cancer, attention has been focussed on differences in gene expression from X chromosomes in men and women with a preponderance of genes involved in immune responses being expressed in women. Important findings have been that some of the genes are important epigenetic regulators that play fundamental roles in immune responses. One of the striking [...]

A rationale to prioritise vaccination of HSCT patients against COVID-19
Leclerc, Mathieu ; Maury, Sebastien
Patients who are frail or have significant comorbidities are especially susceptible to severe acute respiratory syndrome coronavirus 2 infection, and are more likely to develop severe disease, with a higher risk of death than in the general population. This increased risk is notably true for patients with cancer, including those with haematological malignancies. When such patients receive a haematopoietic stem-cell transplantation (HSCT), either allogeneic or autologous, it is possible that [...]

Clinically detected thyroid cancer is rare in children, with one to three cases per1 million children per year. The disease—a papillary carcinoma in most cases—is frequentlymore extensive at diagnosis in children than in adults, particularly in children agedyounger than 10 years who often have large thyroid tumours with lymph node and lungmetastases. The overall prognosis is favourable even in cases of metastatic disease, but thetreatment burden can be substantial, and thyroid cancer related [...]

Trends of Cancer-Related Suicide in the United States: 1999-2018
Han, Xuesong ; Hu, Xin ; Zhao, Jingxuan ; Ma, Jiemin ; Jemal, Ahmedin ; Yabroff, K Robin
The suicide rate has steadily increased in the United States during the past 2 decades. Cancer patients have elevated suicide risk because of prevalent psychological distress, treatment side effects, and potentially uncontrolled pain. Efforts to promote psychosocial and palliative care may reduce this risk. Using the 1999-2018 Multiple Cause of Death database, we found a decreasing trend of cancer-related suicide during the past 2 decades with an average annual percentage change (AAPC) of [...]

Importance : Immune checkpoint inhibitors have been approved for use as a second-line therapy for hepatocellular carcinoma (HCC) in patients who previously received sorafenib. Pembrolizumab has shown substantial antitumor activity and a favorable toxicity profile as a second-line treatment of HCC. However, considering the high cost of pembrolizumab, there is a need to assess its value by considering both the clinical efficacy and cost. Objective : To evaluate the cost-effectiveness of [...]

Chiang et al examine the cost-effectiveness of pembrolizumab for the second-line treatment of patients with advanced hepatocellular carcinoma previously treated with sorafenib, on the basis of the results of the pivotal phase 3 trial KEYNOTE-240. The study finds that pembrolizumab for this indication is not cost-effective at the reference willingness-to-pay threshold in the US of $150 000 per quality-adjusted life-year. This result was consistent even in the optimistic setting, which assumed [...]
